Empowering Senior Women to Age in Place with Dignity

Program Description: The $35,000 grant from the RSFWF supported its mission to help low-income seniors, especially women, live safely and independently in their own homes. Funding enabled the delivery of personalized services including case management, transportation, grocery delivery, home safety modifications, and social support. A majority of the grant used to support personnel, rent and  professional services.

Results and Milestones: All project objectives were met/exceeded:

  • Maintain Independent Living: 95%+ remained in independent living
  • Reduce Social Isolation: 90% felt more connected thanks to staff and volunteer engagement
  • Access to Mental Health Services: 100% met their mental health transportation needs
  • Fall Prevention: 88% of senior women did not experience a fall
  • Wellness Support: 200 in-home visits and 7,000+ phone check-ins
  • Nutrition Assistance: 7,200 bags of produce and pantry staples to 450 seniors
  • Transportation: 12,000 rides for medical, grocery, and social needs
  • Home Safety: 200 safety modifications to prevent fall
  • Housing Stability: 43 seniors supported through our HomeShare Program
  • Information & Referral: 2,300 seniors connected to vital community resources

 

Future Plans: With increased demand, ElderHelp is strengthening its infrastructure through cross-agency collaboration, coordinated training and shared resource systems with Meals on Wheels (fellow RSFWF 2023-24 Grantee), Alzheimer’s San Diego, San Diego Food Bank, Senior Gleaners.These efforts, including the pilot Senior Volunteer Network with Meals on Wheels, streamline volunteer onboarding, reduce waitlists, and close service gaps, thereby allowing more seniors to age with dignity, safety, and independence.
